If you need a name badge, please contact Arch Copeland directly at arch@mmpjax.com or see him at the meeting. Badges are available at no charge. All Directors are encouraged to get and wear them.


 

JAGA Director shirts are available for $30 at the meeting.

For golf, form your own team including you, your Professional, your General Manager and your Groundskeeper.  Other club officials and Assistant Pros are just fine.  Your foursome does not need to fit the above descriptions exactly.  Unattached Directors and Officers will be randomly selected to form other foursomes. 

All players except Professionals must have verifiable handicap indexes.  We are playing our normal best two balls net format.

Meeting: 10:30 am - Lunch: 11:30 am - Golf: 12:30 pm

Fees: Golf and lunch: $65.00 per person
 

      Reservations should be made no later than Thursday, June 7th, 2012 at 6:00 pm. Our host club needs an accurate head count so they can properly prepare for our gathering and the golf pairings must be determined in advance. Late reservations, no-shows, and surprise attendees are unappreciated by all.

      If you make a reservation and later discover you cannot attend, please call and cancel as early as possible. Anyone making reservations later than June 7th at 6:00 pm will be eligible for golf only if foursome fill-ins are needed.

      Make your reservation by calling Adair Roberts at (904) 285-2387 or make them here by completing and submitting the form below.  When you make a phone reservation, please know your current handicap index and your choice of tees (see below for tee information).

 NOTE: If you make a reservation and subsequently need to cancel, you must notify Adair Roberts at (904) 285-2387 by noon on the Sunday preceding the event or you are responsible to pay the applicable fee.

Instructions:

A separate form submittal is required for each person attending, one for you and one for each of your guests.  Complete all applicable fields. 

Note: Home club, handicap index and tee selection are required for anyone who wants to play golf and be in the 'game'.

In the Comments field, please identify your guest by giving their position in your club.

You may NOT change your choice of tees at the meeting.  Choose carefully.
